TITLE VI PART-C

CAREGIVER SUPPORT PROGRAM

 

The purpose of the caregiver support Program is to serve the elders and their families with information of training, along with respite care for the caregiver. The caregiver will be able to take time for them and their personal needs. The training will help them to take better care of their loved ones and themselves. Some caregivers are with their loved ones around the clock and get tired and sometimes feel that no one cares and they get depressed. This is where the support groups are going to help. Native Americans have already been taking care of their parents/relatives who have health and mental problems. This program will be beneficial for them. 

The Our-Reach/Provider will be responsible for providing information to the public and group caregivers at health fairs, through tribal, local and community newspapers and will also develop a Quarterly Title VI Newsletter, which will be posted at the Title VI Nutrition site, Pawnee Indian Health Service Clinic and the Tribal Headquarters/Building 64. In home and individual caregiver information and assistance by telephone or scheduled in-home assessments of needs and help in using services that are available from different agencies will be provided. The possible, to assure that the caregiver receives the services by establishing adequate follow-ups on the requests will be done on each client. Support groups, providing guidance from other caregivers and guest speakers, in home and individual caregivers skills training, and workshops will be utilized.

Since the Caregiver Program is still new to everyone, training will be a big asset for the staff and surrounding areas. We will be in contact with other Title VI programs outside of the tribal

jurisdiction to exchange information coordinate services and will provide referral services. We also have day-to-day contact with the elders. If caregivers can’t get to a support group meeting, the staff will also participate in caregiver skilled training workshops, therefore an in home training will be set up for the caregiver.

The Out Reach/Provider will also perform in home aide/sitter duties on scheduled time lines with a limit up to four (4) hours per week for each individual. This person will be required to do light housekeeping (dusting, vacuuming, or changing bedding) in the client’s room and make the client comfortable by repositioning as needed and changing the client if needed. The Provider will also cook a light meal for that person if the time is scheduled during a meal-time. Providing, the client is not on a special ordered diet.

The Out Reach/Provider will not be allowed to give medication or change bandages forth the clients. This activity will referred to his/her home health care provider. The Title VI Part-C: Caregiver Support Program are as follows: -Title VI Supervisor and Keller Rice-Out Reach/Provider.
